FEES & PAYMENTS

Insurance:

• I am in network with Cigna, Aetna, United and Wellpoint. Please Email me for more information and eligibility.

• If you have a different insurance plan, payment is due at the time of your visit. After your appointment, you’ll receive a superbill that you can submit to your insurance company for possible out of network reimbursement.

• Please not that reimbursement is determined by your own insurance plan and cannot be guaranteed.

• A fee of $175 will be applied with Wildflower due to travel & administration fees. Only for in-home visits. 

Private Payment:

• Payment is required at time of booking to reserve your appointment date and time. To secure scheduled time slot, payment MUST be received within 24 hours of booking.

• Appointments that remain unpaid after 24hours are NOT CONSIDERED CONFIRMED and will be cancelled.

• The Affordable Care Act mandated insurance payers to cover lactation services. You may request a itemized superbill to submit to your insurance for possible reimbursement of any out-of-pocket fees.  It is always advisable to call your insurance company and ask their policy on covering lactation services. 

CANCELLATION POLICY:

• Missed Appointments and/or canceled appointments without at least a 24-hours' notice, will be charged a $100 fee with the credit card on file. 

• If you purchase a package or bundle and choose to cancel, you must submit your cancellation at least ONE WEEK prior to your scheduled appointment. Eligible cancellations will receive a refund for the remaining balance, minus a $75 processing fee.
  

ADDITIONAL FEES:

• Multiple babies (twins, triplets, ect) are an additional $50/baby and require a 1.5-hour initial consult.
